Open Position: Healthcare Navigator/Outreach Case Manager
The Healthcare Navigator and Outreach Case Manager provides mobile case management through street outreach as well as helping homeless clients gain access to healthcare systems. The essential duties of this position include:
Locating, identifying, and building relationships with unsheltered homeless people and engage them for the purpose of providing immediate support, intervention, and connections with homeless assistance programs and/or mainstream social services and housing programs.
Completing assessment of client/families to determine strengths and needs, identifying goals and creating individual services plans for each client and maintaining frequent contact with clients to monitor progress toward goals.
Supporting homeless individuals and families to identify and overcome challenges to accessing the healthcare system by providing care coordination, health education and interdisciplinary collaboration.
Connecting clients to healthcare by working with community partners to facilitate enrollment, helping to gather documentation, completing required paperwork and following up on enrollment progress to ensure that the client is enrolled in community health care services.
Communicating, collaborating with and supporting all programs and staff at Three Oaks Center.
Building working relationships with partnering community resources.

Open Position: Veterans Case Manager
Position Overview: The Supportive Services for Veteran Families Case Manager will provide case management and advocacy to veteran clients assigned to them. The primary focus of this position is to support clients in their efforts to gain housing stability, financial stability, and life skills development to prevent and end homelessness.
Essential Job Functions:
Complete assessment of clients to determine strengths and needs
Recommend goals and develop an Individual Service Plan for each client
Ensure that clients are connected to the correct services in the community to successfully meet each goal
Maintain frequent contact with clients to monitor progress with goals and to ensure compliance with Three Oaks Center’s expectations
Assist clients with finding and maintaining affordable housing to prevent and end homelessness
Work with clients to create reasonable monthly budgets, ensure clients can and do pay monthly expenses and spend within budget constraints
Maintain accurate case files
Utilize database to track and report client data and outcomes
Communicate, collaborate with, and support all programs and staff at Three Oaks Center
Build working relationships with partnering community agencies
Job Requirements and Skills:
Minimum high school diploma
College Degree or 2 years’ experience preferred
Must have reliable transportation
Must be an excellent verbal and written communicator
Must be a proactive critical thinker and problem-solver
